Baltimore County New Student Health Registration Parent or Guardian should complete this form and return it to the school in order to complete registration.
Maryland School's Record of Physical Exam Maryland State law requires that students entering public schools for the first time must have a physical exam.  The physical exam must be done by a physician or nurse practitioner, 9 months before entry or 6 months after entering a Maryland Public school.
Maryland Department of Health and Mental Hygiene Immunization Certificate Maryland law requires schools to obtain proof of immunizations in order for children to attend school.

Maryland Department of Health and Mental Hygiene Blood Lead Testing Certificate

Maryland requires lead poisoning testing for children living in at-risk areas.  Please fill out top 2 sections. Read the lead poisoning letter and make sure your pediatrician tests your child, if you live in an at-risk area, and records the results on the immunization form provided.  If you have never lived in an at-risk area, sign the exemption part of the letter.

Parent's Request to Administer Medication at School This form is required for our nurse to administer physician prescribed medications.  It must accompany any prescription medications that are sent to school.  It is only effect for the duration of the prescription.
Consent for Approved Discretionary Medications This form is required for our nurse to administer "over the counter" medications to your child, such as Tylenol or cough drops.  This form does not require a physician signature, only yours.  This form will remain in effect for the full school year.
